Pt. B. D. Sharma University of Health Sciences, Rohtak, has released an updated list of documents required for document verification for candidates seeking admission to MBBS/BDS courses in Government, Government Aided and Private Medical/Dental institutions in Haryana for the 2026–27 academic session. The notice is dated September 9, 2026.

Candidates are required to bring the original documents along with one set of photocopies before the State Admission Committee for verification.

Documents Required for Verification

Candidates should carry the following documents, as applicable:

1. Matriculation/High School/ICSE/Class X/Class XII certificate or equivalent proof of date of birth.
2. Three recent passport-size photographs.
3. Character certificate from the Head of the last institution attended or First Class Executive Magistrate.
4. Haryana Resident/Bonafide Resident Certificate, wherever applicable.
5. Haryana Caste Certificate, wherever applicable.
6. Dependent of Freedom Fighter Certificate along with the prescribed undertaking, wherever applicable.
7. ESM Priority Certificate issued by the Zila Sainik Board and countersigned by Rajya Sainik Board, Haryana, Panchkula, wherever applicable.
8. Creamy/Non-Creamy Layer Certificate for BC-A/BC-B candidates. For 2026–27, the certificate should be issued on or after April 1, 2026.
9. Income & Asset Certificate for EWS candidates for 2026–27, issued on or after April 1, 2026.
10. Disability Certificate issued by notified NTA centres, wherever applicable.
11. NEET UG 2026 Result Card.
12. Valid identity proof such as Aadhaar Card, PAN Card, Driving Licence, Passport, Voter ID, Ration Card or Parivar Pehchan Patra.
13. Printout of the application form and proof of payment of the provisional tuition fee.

Additional Documents for Specific Categories

Candidates claiming benefits under the PMSS (Post Matric Scholarship Scheme) must produce their Parivar Pehchan Patra.

Candidates who passed Class 12 from outside Haryana but wish to claim Haryana residency benefits must submit an affidavit from their parent/guardian, duly attested by an Executive Magistrate, Oath Commissioner or Notary Public, confirming that neither the parent/guardian nor the candidate has availed the benefit of residence in another state.

Candidates allotted seats under the Minority category must also submit an affidavit establishing their minority status.

Candidates should ensure that all applicable certificates are valid and carry original documents plus one photocopy set at the time of verification.
